A written request should contain the name, address, and telephone number of the person seeking to view the records, as well as which records are being sought. Written requests include an email or a fax.
Copies of records may be given to a person who has requested inspection. Reasonable fees may be charged, up to 25 cents per page. Advance payment of fees may be required before copies are made.
