Mi Via Living Supports
Mi Via Living Supports are individually determined supports that help the participant to stay in his/her own home and community. These supports can provide needed assistance with the activities of daily living, home management, supports for health and safety as well as independent living skills. Supports can be provided using there (3) different model: Home Health Aide, Homemaker/Direct and In-Home Living Supports

Home Health Aide
Home Health Aide services provide total care or assist a participant 21 years and older in all activities of daily living. Home Health Aide services provide total care or assistance to the participant in a manner that will promote and improve the participant’s quality of life and provides a safe environment for the participant. These services can be provided outside the participant’s home. Home Health Aide services are only provided through agency provider. This service is not to be provided in homes or apartments owned or leased by provider agencies.
Homemaker/Direct
Homemaker/Direct Support Services is available to participants who are 21 years and older. The service is provided on an episodic or continuing basis to assist the participant with activities of daily living, performance of general household tasks, provide companionship to acquire, maintain, or improve social interaction skills in the community and enable the participant to accomplish tasks. Homemaker/Direct Support services are provided in the participant’s own private home and in the community, depending on the participant’s needs and choice. This service is not to be provided in homes or apartments owned/leased by provider agencies.
In-Home Living Supports
In-home Living Supports are available to participants who are 18 years and older and are individually designed services and/or supports that are related to the participant’s qualifying condition or disability. This service is not to be provided in homes or apartments owned or leased by provider agencies. This service is provided on a regular basis at least four (4) or more hours per day, one or more days per week. The number of support hours are based on participant needs and may be up to twenty-four (24) hours per day. Hours and days must be specified in the service plan. In-home living providers must assure twenty-four (24) hour response capability to address scheduled or unforseen needs, health, safety and security concerns. Service coordination and nursing services are not included in this service.
In-home Living Support Services are paid at a daily rate. They may not be provided by hourly employees. If the participant wishes to hire an hourly employee to perform this type of service, he or she would utilize Homemaker/Direct Support Services instead.
